Which Ultrasound Lifting Devices Hurt the Least

Comfort during ultrasound lifting of the neck and chin is set by treatment depth, energy per line and cooling. Here is how the main platforms compare, and what a provider can do to make it bearable.

The short answer


Comfort in ultrasound lifting is decided by depth, energy per line and cooling — not by brand marketing. Mid-dermal platforms treating around 1.5mm with an integrated cooling handpiece are the easiest to tolerate. Deep microfocused passes at 4.5mm along the jaw and under the chin are the most painful step in any protocol, on any machine.


Know the types, ranked by comfort


Mid-dermal parallel-beam ultrasound (Sofwave). The most tolerable of the group. It delivers into the mid-dermis at roughly 1.5mm and cools the skin surface through the handpiece during delivery. Most patients describe brief heat that fades within a second of the pulse. It is the sensible starting point for anyone who has previously abandoned a deep treatment partway through.


HIFU platforms with shallow cartridges (Ultraformer and similar). Comfort here is a setting, not a property of the machine. Treated with 1.5mm and 3.0mm cartridges at moderate energy and a high line count, these are well tolerated. Switch to a deep cartridge along the mandible and the experience changes completely. Ask which cartridge depths the provider plans to use on the chin before you agree to a protocol.


Microfocused ultrasound with visualisation (Ultherapy). The most uncomfortable and, on deep laxity, the most capable. The 4.5mm transducer is what produces the lifting effect and it is also what produces the pain. Newer transducer generations deliver lines faster and allow protocols with lower energy per line at a higher line count, which is meaningfully more comfortable than the original settings, but this is still the treatment people describe as an intense deep ache.


Full-field radiofrequency alternatives. Worth naming as the comparison, because patients frequently choose them on comfort grounds. Radiofrequency spreads heat through a volume rather than concentrating it at points, so it feels like a strong warm massage instead of individual jolts. It tightens rather than lifts.


Why depth drives the pain


Focused ultrasound works by converging sound energy at a fixed distance below the surface until a tiny volume of tissue reaches the temperature at which collagen denatures. That focal point is deliberately small, so all of the sensation is concentrated. The deeper the focus, the more tissue the beam passes through and the closer the deposit sits to structures that register it strongly.


Under the chin and along the jaw, a 4.5mm focus lands close to the periosteum covering the mandible. Bone conducts the pulse efficiently and the surrounding area is densely innervated, which is why the same energy that feels manageable on a cheek feels sharp along the jawline. That is also why patients often report the treatment as tolerable for the first half and difficult for the last quarter — the deep jaw and submental lines are typically saved for last.


Two other variables change the experience more than most people expect. Energy per line is adjustable: the same total dose can be delivered as fewer high-energy lines or more moderate ones, and the second version is considerably more comfortable. And surface cooling helps only for shallow work, since it cannot influence a focal point several millimetres down.


Benefits and the honest trade-off


The benefit of choosing a more comfortable platform is not just a nicer afternoon. Patients who tolerate the full protocol get the full protocol — treatments abandoned or under-dosed because of pain deliver visibly less. If pain is the reason you would decline treatment altogether, a mid-dermal device you will actually complete is the better clinical choice.


The trade-off has to be stated. Comfort usually comes from treating shallower, and shallow treatment does less for deep laxity. If your concern is genuine submental sagging, a comfortable mid-dermal session will improve skin quality and disappoint you on lift. If your concern is fine crepiness and early softening of the jaw border, the comfortable option is also the correct option. Choose by the layer that is failing, then optimise comfort within that choice.


Making the appointment more comfortable, and what follows


Practical steps that work: take an oral analgesic about an hour beforehand if your provider approves it, eat first, avoid caffeine that morning, and avoid scheduling in the days before a period, when pain thresholds are lower. Ask the provider to start on the cheeks and finish the jaw and submental lines in short blocks with breaks, and to lower energy and add lines if you are struggling rather than pushing through. Say something the moment a line is genuinely painful — a sharp, electric sensation is different from deep heat and is worth flagging immediately.


Afterwards, expect tenderness on pressure along the jaw for a few days, occasional dull aching or a tight feeling for one to two weeks, and possible small areas of numbness or tingling that resolve without treatment. Mild swelling and redness settle within hours to a couple of days. Take simple pain relief if needed, use sunscreen, and go back to normal activity the same day. Bruising along the jaw is uncommon but not alarming. Contact the clinic if pain worsens after the first week, if you notice asymmetry when you smile, or if numbness spreads rather than shrinks.

Which part of a neck and chin treatment hurts most?

Is a less painful device also less effective?
Often, in a specific sense. Comfort usually comes from treating shallower, and shallower treatment does less for deep laxity while doing more for skin texture. The right question is not which hurts least but which reaches the layer causing your problem.
Can the energy be turned down if it is too painful?
Yes, and good providers titrate throughout the treatment rather than pushing a fixed setting. Lowering energy while increasing the number of lines maintains the total delivered while spreading it out, which is more comfortable and still clinically useful.
Does it hurt afterwards as well?
Mildly. Expect tenderness to the touch along the jaw for a few days and an occasional dull ache or tight sensation for one to two weeks. Transient numbness or tingling can occur and resolves on its own. Persistent or worsening pain warrants a call to your provider.
